The LPN is accountable for delivering patient care to the adult/geriatric patient population with a varying diagnosis and treatments. The LPN carries out assignments under the direction and with minimum supervision of the registered nurse. The LPN is assigned duties that require a high degree of skills and accuracy in performing technical procedures using powers of observation, perception and judgment.Learn more about this agencyResponsibilitiesThe LPN will:Ensure proper identification of patient/resident per policy before administering care.Assist co-workers willingly without having to be asked, and present an attitude that promotes unified teamwork.Function in collaboration with the registered nurse in identifying needs of patients and carrying out assignments.Demonstrate keen observation skills with accurate reporting and recording of significant changes in patients' conditions and actions taken.Assist the RN with admission of a patient.Function as one-to-one nurse in Primary Care under the direct supervision of the NM/designee.Recognize and communicate with NM/team leader and/or designee the need for additional instructions for self and those under their direct supervision. Assists in data gathering for patient education needs, suture removal wound care/dressing, wound cultures, diagnostic skin testing and Unna boot application.Demonstrate the ability to utilize the computer independently, for obtaining patient information and maintaining accurate input of data.Treatment/exam room maintenance (infection control, supplies stocked, equipment operational).Assist with gynecological procedures as indicated, such as pap smear, colposcopy, transvaginal ultrasound, loop electrosurgical excision procedure (LEEP), biopsies and minor surgical procedures.Practice safety precautions according to written procedure in performance of duties when working with patients and patient's personal care devices, medication administration and unit equipment.Perform duties in a safe manner to prevent patient/resident/employee accident/injury.Safely operate equipment such as specialized beds, prosthetic devices and suction machines.Implement proper body mechanics when utilizing mechanical lifting devices and other transfer aids that assist in lifting and moving patient/residents, while preventing injury to self and others.Assist in maintaining a clean and orderly unit environment; keep supplies and equipment clean and in proper places, observe infection control guidelines, etc. The LPN is expected to take personal accountability in reducing patient's risk of trips and falls by taking quick and immediate action to correct any environmental safety concerns.Utilize principles of infection control, cleanliness and hand washing to assure a safe, clean environment to prevent cross infections.Understand and utilize principles of Universal Precautions and type of isolation for protection of patient/residents and/or self.Report faulty equipment or safety hazards immediately to supervisor.Be knowledgeable and correctly utilizes the RACEE (Rescue, Alert, Confine, Extinguish, Evacuate) procedure during fire drills and emergency situations. Being able to correctly verbalize the steps within the RACEE procedure is the employee's responsibility.Verbalize role of unit/staff in a disaster including recognition of disaster alarms/notification per policy. Also recognizing alternate power sources and mechanism to use on the event of utility system failure.Be knowledgeable regarding location of the Material Safety Data Sheet (MSDS) binder of all hazardous chemicals used within the assigned area and take appropriate action as needed to mitigate situation.Document medications administered, care and treatments provided and patient/resident's response to therapy in the clinical record as ordered.Recognize and report acute changes in patient conditions to the provider and documents observations appropriately.Maintain good rapport with patient/residents, families, volunteers, visitors, and co-workers.Accurately document measurements such as vital signs, weights, and finger sticks for glucose when ordered.Provide for patients' physical and psychosocial needs, including emotional support to patient/residents and families.Assist with diagnostic procedures such as physical exams, ensuring the use of sterile technique when appropriate.Collect and properly label specimens for laboratory study such as urine, sputum, and stool. The position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This also includes frequently pushing medication carts, wheelchairs, lifts, and beds. The LPN must be able to care for Veterans with various medical and psychiatric issues. At times, these Veterans exhibit unpredictable behavior. The LPN must be able to work closely with others, have sufficient hearing to respond to various alarms, and must be able to communicate verbally and in writing. They will make caring for the Veteran their priority while in the work setting, and demonstrate customer service principles in all aspects of work.The LPN is subject to exposure of communicable diseases. The use of personal protective equipment such as, but not limited to, gloves, facial/eye protection, and protective clothing is required.
