Monitored Crisis Detox LPN JOB SUMMARY - The Level II Residential
LPN is responsible for the assessment and management of withdrawal
signs/symptoms of clients presenting to the unit. - These include
initial medical assessments of clients during intake, and providing
appropriate nursing care form Medical Detox, Medically Monitored
Detox and Residential Services. JOB DUTIES/RESPONSIBILITIES This
job description is not intended to be all-inclusive; and employee

employment. 1. Review of admission and discharge criteria before
admission t the unit and refer to Coordinator/Medical Director as
appropriate. Perform the initial medical assessment of individuals
within 2 hours of admission. Makes a determination of the
appropriate detox protocol upon admission. 2. Complete all
documentation in compliance with CARF and program requirements.
Maintains accurate records of nursing interventions. Monitoring of
treatment plan effectiveness and compliance, including obtaining
consents. Review with client ASAM placement criteria and obtain
signature Q shift. Perform TB testing on each client upon admission
and document results 48-72 hours later. 3. Monitor appointment
calendar and available beds each shift/day to maintain full status.
Upon admission, determine client's discharge date/time and place on
board. 4. Maintains accurate records of nursing interventions.
Administer prescribed, routine and PRN medications as scheduled no
more than 1 hour ac or 1 hour pc of prescribed time. Perform
charting 3 X per 8 hour shift, 4 X per 12 hour shift and PRN .
Complete nursing assessment within 2 hours. Monitor Clients' VS/
withdrawal signs/symptoms per CIWA/COWS upon admission, shift X 3
days then QD if score is >10. Monitor significant changes in the
clients' condition Q shift and make appropriate referral. Ensures
that physicians' orders are evaluated by nursing staff within 2
hours of the order. Completes nursing assessments within time
allotted, III.7D/MMCD and RR5 ? 2 hours. Makes client rounds and
notes problems for follow-up. Assist clients in obtaining
prescribed medications. Maintains harmonious, productive
interpersonal relationship with clients, co-workers, visitors and
families. Ability to complete all work assignments within shift
assigned. 5. Monitoring of treatment plan effectiveness and
compliance, including obtaining consents. Obtain Medication
Consents on all medications upon admission/PRN prior to medicating
client. Completes the required TX plan with client upon admission.
Monitor electronic record for completed documentation. 6. Functions
as a member of a multi-disciplinary management team. Attends shift
reports each shift with IC/OG nursing staff without tardiness.
Participates in call rotation as scheduled, returns calls within 15
min. Attends all mandatory staff meetings monthly. Must be able to
perform shift work as needed to include overtime, weekends and
holidays. Must be punctual, maintain a good attendance record. Able
to maintain a level of supervising and capable of following and
interpreting policy and procedures as they relate to the clients
and staff in the detox unit. Ability to relate positively and be
emotionally supportive to individuals who are chemically
dependent/co-dependent. Able to establish professional client
boundaries, effectively sets limits with clients. Observe clients
for changes in physical/mental conditions and documents changes for
consultation with the Medical Director/Medical Services
coordinator. Nursing Staff will keep other staff members informed
on status of the clients. Performs other duties as assigned by the
Medical Service coordinator. COMPENSATION: - Starting salary for
this position is approximately $22.99 /hr based on relevant
experience and education. QUALIFICATIONS - Medically Monitored
Crisis Detox LPN Education / Knowledge: - Graduate of an accredited
Licensed Practical Nurse Program and possess a current nursing
license. - Possess a working knowledge of basic nursing techniques.
- Demonstrates ethical and appropriate behavior for a clinical
environment to include, but not limited to, dignity, patient's
rights and privacy, and the maintenance of confidentiality of
patient information. - The ability to retrieve, communicate and
present data and information both verbally and in writing required.
- LPN with current Tennessee license and experience in psychiatric
nursing preferred. - LPN certification as Psychiatric/Mental Health
Nurse preferred. - Should have sound knowledge of psychiatric
medications, diagnoses, treatment modalities and interventions. -
Ability to use computer. - Must be CPR certified or willing to
become certified. - Will also need to complete the required Handle
With Care Training. - Assist and evaluates the need for emergency
transfer to hospital in cases of serious injury, illness. -
Knowledge of substance abuse, chemical dependency, co-occurring
disorders and recovery. - Ability to reason and make judgments
regarding client's care. - Must be flexible with work scheduling. -
Participates in On-Call rotation as scheduled. - Class room
training in Pharmacology, medical terminology, charting and nursing
process. Experience: - Prefer 1 years' experience in substance
abuse and treatment techniques. - Experience in psychiatric nursing
preferred. - Experience in working with special populations
including individuals with Substance Use/Abuse and/or Addiction,
Severe and Persistent Mental Illness, Co-Occurring Disorders, and
Dual Diagnosis Disorders preferred. - Computer experience is
helpful. - Experience working in a crisis setting preferred.
Physical: - Exposure to biological hazards. - Hearing of normal and
soft tones. - Close eye work. - Lifting up to 50 lbs. - The
individual must also be able to maneuver throughout halls, clients'
rooms and surrounding facility in response to emergencies. - A
significant amount of writing is required. - Exposure to biological

Valid driver's license- F endorsement. - Lifting up to 50 lbs. -
Pushing/pulling up to 150 lbs. - Frequent sitting, standing,
walking, bending, stooping, and reaching. - Required to be
certified in, and effectively implement, verbal and physical
de-escalation techniques that include a wide range of bodily
movements; including but not limited to, grasping, holding another
person, going down on knees, running, and walking. Location: - Knox
