Princeton Transitional Care & Assisted Living is a partnership between Signature HealthCARE and Ballad Health bringing a new level of health care services to the Tri-Cities area. The new Princeton Transitional Care will provide a new home for skilled-nursing care and will add 60 assisted living apartments, 47 short-term skilled-nursing and rehab private suites. The state of the art community will feature a streetscape with a chapel and theatre, library and media center, Blue Ridge market, coffee bar, ice cream & bistro, billiard parlor, outdoor walking paths, and on-site physician office.The new one-story, 73,000-square-foot facility will be located on Princeton Road on the site of the former North Side Hospital.A growing number of our centers are earning quality assurance accreditation and pioneering person - directed care. Many of our skilled nursing facilities have achieved a 4 or 5 - star overall rating from the Centers for Medicare & Medicaid Services. Additionally, we have been awarded as a certified Great Place to Work for three years in a row and Modern Healthcare's "Best Places to Work"!Signature Healthcare is a long-term healthcare provider that is committed to providing an environment of wellness, healing and independence for its residents.How you will make a differenceManage the facility's admissions process and provide target audience with facility-related informationCollaborate with the facility team in achieving census and quality mix goalsHow you will spend your timeMeet the physical and sensory requirements stated below and be able to work in the described environmentRespond to inquiries from hospital discharge planners, families, and other referral sources to include e-referral systems, tours, and phone inquiriesEnsure positive admissions experience by communicating admissions with appropriate stakeholders and completing room readinessUnderstand census, quality mix, and bed management needs of the facilityUtilize the Customer Relations Management system by entering leads from referral to closingMaintain a working knowledge of Federal and State regulations, referral processes, and reimbursement (Managed Care, Medicare, and Medicaid)Manage the inquiry process professionally, timely and with appropriate follow-upComplete admissions paperwork according to company policiesAssist with facility business development eventsOther special projects and duties, as assigned The qualifications you will needHigh school diploma or GED required or equivalent related work experienceMinimum of one (1) year experience in healthcare preferredDemonstrated basic to intermediate skills in Microsoft Office SuiteMust have highest level of professionalism with the ability to maintain confidentialityAbility to communicate at all levels of the organization and work well within a team environment in support of company objectivesCustomer service oriented with the ability to work well under pressureStrong attention to detail and accuracy, excellent organizational skills with ability to prioritize, coordinate and simultaneously maintain multiple projects with high level of quality and productivityStrong analytical and problem solving skillsAbility to work with minimal supervision, take initiative and make independent decisionsAbility to deal with new tasks without the benefit of written proceduresApproachable, flexible, and adaptable to changeAbility to function independently, have flexibility, personal integrity, and the ability to work effectively with stakeholders and referral sourcesSignatureHealthcare is an Equal Opportunity-Affirmative Action Employer - Minority / Female / Disability / Veteran and other protected categories.
